We offer our bariatric evaluations in one of our office or online. Our online evaluations are 100% HIPAA compliant, convenient, and secure.
Scheduling Appointments
You have two options to schedule your appointment.
- Call the office at 973-696-0800.
- Use our online patient portal and follow the instructions in the portal.
Prior to your appointment you will receive an email with links to the forms and psychological testing that need to be completed prior to your appointment. You will also receive the link for the secure video platform. The platform can be used with any smart phone, computer, or tablet.
